Athletics Online Clearance and Forms
ONLINE CLEARANCE INORMATIONAL POWERPOINT
Follow these steps To complete online clearance
STEP 1 - Create account at www.homecampus.com/login
Students - please do not use your hlpnet.net account. Use your personal email
- Create an account
- Verify your email account
- Log in with username and password
- Select "New Clearance"
- Complete all required fields
STEP 2 - Physical Form, Proof of Health Insurance - Print and Upload to athleticclearance.com (Physical Form and Insurance Form)
If you are not covered under a medical plan you can purchase through the following:

STEP 3 - HLPUSD COVID Return to Play Forms - Print both and Upload to athleticclearance.com (Covid Guidelines and Covid Protocol Forms)
Email Athletic Secretary Elexus Gonzalez (egonzalez@hlpusd.org) and Athletic Director Willie Allen (wallen@hlpusd.org) once your account says "Pending". If your account does not say pending they you are not ready to be cleared.
